If there is no certifier document in the names.nsf you probably didn't create a certifier ID.
You have to create a certifier ID for your OU (and your O of course), after that you can certify a server with a certifier ID.
If you manually create a certifier document, you need to know the public key of the existing certifier ID and put it into the doc.
Note that you cannot create certifier IDs with the CA process.
